COVID Vaccine Clinic

for kids ages 5-11, kids 12+, and adults

Tuesday, December 14th

1:00 pm - 3:30 pm

West Hill Elementary School
Parents MUST be present

**Please bring your ID, and insurance cards with you. Insurance is NOT necessary.

Clinic is a walk-in, no scheduling necessary.**

You will be required to park in the lower lot off of North Irvine Avenue.
Once you park in the lower lot, you will need to walk up to the gym

entrance off Ellsworth Avenue.

**Second Dose Thursday, January 7th from 2-4pm at West Hill Elementary**
***Vaccines will be available to adults to be vaccinated or for their booster***
